Myofascial Release of Southern Illinois provides out-patient Occupational Therapy.  Prior to coming to your first session, you will need to obtain an order for Occupational Therapy from your physician, nurse practitioner, physician assistant, dentist, or podiatrist.  The order can be faxed to the clinic or you can bring it with you on your first session.  In addition to your order, please bring your insurance card (if we will be billing your insurance). Please complete the available new patient forms and bring them with you.
